Health & Safety Issue Observed	insufficient means of communication (ie: PA, radio, etc.)
Health & Safety Issue Details	It is highly recommended to have the Main Office extension posted on or near the phone. This is for OTs who may not know what the Office extension is, in case of an emergency. The following rooms were noted to require the Main Office extension: 108, 112, 170A (science prep room), 174, 173, 171, 107, 105. No doubt, there will be other rooms requiring the Main Office extension to be posted.

Corrective Action	Principal to review with Family Team Leader to ensure a communication plan is in place. PA's and telephones are not required in 'service rooms'.
