Health & Safety Issue Details	Heat exhaust in Boiler room. No cover over hole in the exhaust to outside in cold weather the temperature drops below zero in the room.pipes have burst. Potential Worker hazard. Kindly install a cover not keep extreme cold outside of room. Burst pipes are a slip and fall hazard.

Corrective Action	Principal to tag out equipment until repair is complete or remove equipment from the workplace.
Corrective Action Taken	opened notification 13962002 requesting the boiler room be investigated to see what can be done to make this room warmer and still satisfy the needed air for boilers. Also to try and ensure no broken water pipes on very cold days in this boiler room.
