Health & Safety Issue Observed	extension cord/power bar - inappropriate use of extension cord/power bar (be specific)
Health & Safety Issue Details	Extension cord used as permanent wiring. Notice for Principal: Please remind staff that extension cords cannot be used as permanent wiring, and if there's no other choice but to use them throughout the day, they will need to be unplugged at the end of each working day. Also, the cord is a potential trip hazard. Currently, the cord sits below the carpet. This actually creates an invisible" trip hazard. The cord should never be placed under the carpet for the aforementioned reason and the fact that there will be unnecessary wear and tear on the carpet while it's walked over daily. Two solutions: unplug cord at the end of each day; and use a wire/cord trough throughout the day (but overtop of the carpet - not underneath). See photo."
